How to Request a Passport Name Change After Divorce
If you change your name after getting divorced, you have to edit the information on your passport and other forms of identification to reflect the change. A passport name change is quick and simple to do once you know how it’s done.
If you would like to change your name by mail, you must be in possession of your most recent passport. If your passport was issued over a year ago, you have to mail in your current passport, a government-issued document showing your name was legally changed, a completed form (DS-82) found online, the “renewal” fees and a recent, color photograph.
If your passport was issued to you within the past year, you need to mail in the passport, a completed form (DS-5504), and a government-issued document evidencing the change.
If you would like to make the request in person, be sure to bring documents that evidence your birth name, new name change, and divorce, as well as money to pay fees and a recent photograph.
